You may:

• Complete the office referral form and submit with demographics and relevant clinical notes, or

• Provide a demographic sheet with a brief written summary including:

  • Referral question

  • Presenting symptoms

  • Onset and course

  • Relevant medical, developmental, and psychosocial history

All referrals are reviewed by clinical staff to confirm medical necessity.

• If approved, the patient will be contacted for scheduling.

• If additional information is needed, your office will be contacted.

• If concerns are better addressed through another service, recommendations will be provided.
