Getting Started

Neuropsychological testing requires a referral and several preparatory steps before scheduling. The information below outlines how referrals are reviewed, how appointments are scheduled, and what documentation is needed prior to evaluation.

The Neuropsychological Evaluation Process

We want families to feel informed and comfortable every step of the way. Here’s what to expect when seeking neuropsychological testing:

New Patient Forms

  1. Choose one: Consent for Treatment (Adult) or Consent for Treatment (Minor)

  2. Acknowledgment of Privacy Policy (HIPAA Notice of Privacy Practices)

  3. Electronic Communication Policy

  4. Patient History and Demographics Forms

  5. Consent for Release of Information - please complete for any providers or caregivers that you would like us to be able to communicate with about the evaluation. 

These forms help us understand your concerns and ensure we provide the most accurate and helpful evaluation possible.

Once completed, you can submit them to the office via email (contact@ak-neuro.com) fax (907-331-0114) or drop them off at our office. 

Important Billing Information

Alaska Neuropsychology, LLC and Dr. Cavanaugh cannot bill insurance for:

  • Academic or achievement testing (e.g., highly gifted program admission)

  • Disability determinations

  • Forensic or legal consultations

If you are seeking one of these services, you are welcome to contact our office to:

  • Discuss self-pay arrangements, or

  • Request a referral to a provider who offers the specific service you need.

Insurance Billing Code Information

96116 – Neurobehavioral status exam (clinical interview and cognitive assessment by the neuropsychologist to determine testing needs and diagnostic considerations).

90791 – Psychiatric diagnostic evaluation (clinical interview focused on emotional, behavioral, and mental health history and symptoms).

96132 / 96133 – Neuropsychological evaluation services (integration, interpretation of test data, diagnostic formulation, and report writing by the neuropsychologist).

96136 / 96137 – Test administration and scoring by the neuropsychologist.

96138 / 96139 – Test administration and scoring by a technician, under the neuropsychologist’s supervision.
